Soy Protein Diet Benefits People with Type 2 Diabetes

A diet rich in soy protein may have lasting beneficial effects for type 2 diabetics, specifically for their heart, blood vessels, and kidneys, according to a study published in the journal Diabetes Care.

This appears to be the first study that ever showed that these beneficial effects of soy protein can remain “stable through long-term consumption.” Dr. Leila Azadbakht and colleagues from the Isfahan University of Medical Sciences closely followed 41 patients for four years. Twenty of the patients were asked to consume a diet that was 35 percent animal protein, 35 percent textured soy protein, and 30 percent vegetable protein. The remaining 21 patients who served as controls were asked to consume a diet consisting of 70 percent animal protein and 30 percent vegetable protein. This control group received the same treatment as the soy group; however, the only difference was the absence of soy protein.

Results showed that those in the soy group had significantly lower levels of fasting blood-sugar, total cholesterol, LDL cholesterol (“bad” cholesterol), and triglycerides. Also, circulating C-reactive protein levels, which signal inflammation in the body, and levels of well-known urinary markers, were also reduced.

Nursing Management of the Common Side Effects of Chemotherapy

A. Bone marrow suppression leads to:

1. Decreased WBC count (immunosuppression).

Avoid crowds; people with infections and small children when WBC count is low
Consume a low-bacteria diet. Avoid overcooked meat and raw fruits and vegetables

2. Decreased platelet count (thrombocytopenia)

When shaving use ellectirc razon only
Avoid contact sports and other activities that may cause trauma
If trauma does occur, apply ice to the area and seek medical assistance
Avoid dental work or other invasive procedures
Avoid aspirin and aspirtin-containing products

3. Decrease hemoglobin and hematocrit (anemia).

B. Gastrointestinal effcts : anorexia, nausea, vomiting, and diarrhea
1. Client education for mainting adequate nutrition , and fluid and electrolyte balance

Eat frequent, small, low-fat meals
Avoid spicy and fatty foods
Avoid extremely hot foods

Nursing management of inadequate nutrition and fluit and elctrolyte imbalances

Administer antiemetics prior to chemotherapy
Weigh client routinely and monitor for weight loss

C. Stomatitis (infammation of the mouth) and mucositosis
1. Client education for stomatitis and mucositosis

Use a soft toothbrush. Mouth swabs may be needed during an acute episode
Avoid mouthwashes containing alcohol. do not use lemon glycerin swabs or dental floss
Consider using chlorhexidine mouthwash (Peridex) to decrease risk of hemorrhage and protect gums from trauma
For Xerostomia, apply lubricating and moisturizing agents to protect the mucopus membranes from trauma and infection
Consicder using “artificial saliva” and hard candy or mints to help with dryness of the mouth
Avoid smoking and alcohol, which can further irritate the oral mucosa

Ankle Mobility Impaired in Diabetes

A new study has proven that people with long-standing diabetes are prone to impairment of their ankle function despite not having peripheral neuropathy. Peripheral neuropathy, which is linked to diabetes, is a progressive deterioration of nerve function in the extremities that can cause chronic pain, numbness, weakness and tingling sensations in the feet.

Under controlled conditions, Dr. Claudia Giacomozzi and her team from Istituto Superiore di Sanita in Rome, tested the muscle performance and ankle mobility of 46 diabetic patients with and without nerve damage and in 21 control subjects. Results showed that compared to the controls, ankle mobility and flexing ability were impaired in all of the diabetic patients regardless of the presence or absence of nerve damage. Researchers said that these findings may help design programs aimed to prevent foot and ankle problems in people with long-standing diabetes. They also called for further studies to fully understand the mechanisms responsible for the foot-ankle biomechanics in diabetics. The study was reported in the journal BMC Musculoskeletal Disorders.

5th AO Trauma Course – Principles in Operative Fracture Management

Over the past 5 years, AO {Arbeitsgemeinschaft fur Osteosynthesefragen-Association for Osteosynthesis / Association for the Study of Internal Fixation (ASIF)} Courses have been held in the country as a joint project of the Department of Health, The Philippine Orthopedic Center, The Philippine Orthopedic Association and the AO Foundation Education with the objective of upgrading musculoskeletal healthcare through the use of modern concepts of operative fracture management. This 3-day course consists of a series of lectures, practical saw-bones work shops and interactive small group discussions.
AO Foundation is a a Swiss-based international group founded more than 50 years ago that run courses of such nature all over the world with the said objectives, including research in fracture biology and orthopedic implants technology, documentation and classification as well as education through courses and fellowship training programs. They have conducted 4 previous courses since 1996 and the last one was held last year at the St. Luke’s College of Medicine.

This year, they will be conducting once again another course – The 5th AO Trauma Course – Principles In Operatinve Fracture Management on April 30-May 3,2010. This is another historic milestone as it will be held in Chong Hua Hospital in Cebu City, the first time they will be having such a course outsite of the Metro-Manila area.
